In consideration of being permitted to volunteer with For His Glory Ministries, I, the undersigned volunteer, knowingly and voluntarily agree to the following:
1. Assumption of Risk. I understand that volunteer activities — including food drives, outreach, events, and community service — may involve physical activity and inherent risks. I voluntarily assume all such risks.
2. Release of Liability. I release, waive, and discharge For His Glory Ministries, its leaders, staff, and fellow volunteers from any and all liability, claims, or demands for injury, illness, or property damage arising from my volunteer service, to the fullest extent permitted by law.
3. Medical Authorization. In the event of an emergency, I authorize the Ministry to seek medical treatment on my behalf, at my own expense.
4. Media Release. I grant permission for photographs or video taken during volunteer activities to be used for the Ministry's outreach and promotional purposes.
5. Conduct. I agree to act respectfully, follow all safety guidelines, and serve in keeping with the Ministry's values.
